$15 CAD rebate on select Waterpik products:  This offer expires December 31, 2027.

Offer valid on any Waterpik Water Flosser with a retail value over $54.99 CAD purchased at retail. 

To receive your rebate:


- Provide your name, address, phone number and email address by mail or online at www.waterpik.ca/rebate.  
- Provide the original purchase receipt:  Circle the Waterpik product purchased, the purchase date, and the purchase price. 
- Enclose the original UPC (purchase seal) from the product packaging.

Refundable rebate amount is $15 CAD Dollars paid via rebate cheque or EFT (Electronic Funds Transfer).

Required documents must be received by January 31, 2028. Limit two rebates per household, name and address.  Offer cannot be combined with other offers.  Each submission must include one form per individual product.  Offer valid from January 1, 2026 to December 31, 2027.  Valid for retail purchases in Canada only.  Offer void where prohibited.  Please allow 8 to 10 weeks for delivery.  Reproduction of this form is strictly prohibited.  Any other use of this form constitutes fraud.
