HOW TO CLAIM YOUR REBATE: 
1. Refund request must be received within 60 days of the purchase date on the sales receipt. 
2. Complete the official online claim form which includes purchase date on the original, store-identified and itemized sales receipt.  No PO Box addresses, only street or rural addresses are acceptable.  Use of multiple addresses to obtain additional refunds is fraud and may result in prosecution.  Multiple submissions will not be acknowledged or returned. 
3. Upload a picture or scanned image of the entire original sales receipt. If the receipt is too long to capture in one picture or scanned image, you may upload more than one attachment. 
4. Upload 1 of the following: 
- a picture or scanned image of the Universal Product Code (UPC number) found on the packaging 
- a picture or scanned image of the coupon 
5. Once we receive your request with all required information, it will be processed and your rebate cheque will be issued within 6 to 8 weeks.

TERMS AND CONDITIONS:  Valid only in Canada.  Rebate offer applies to the purchase of VenixxaTM.  Limit:  one coupon per purchase and not valid on any other offer, sample, trial size or returned product.  Not redeemable for cash.  Facsimile prohibited.  Acceptance of a coupon is based on the laws and regulations of each province.  Void where prohibited by law.  This offer, extended to consumers, requires that the purchase be made by the person redeeming the offer.  Valid to expiry date on coupon.  No submissions will be accepted from retailers, clubs, groups, organizations, wholesalers or contractors.  Refund request must be received within 60 days of the purchase date on the sales receipt.  Please allow 6 - 8 weeks for fulfillment.  Servier Canada Inc. will not be responsible for claims lost, damaged, misdirected or delayed in the mail.  Please keep copies of all documents submitted.  Submissions become the property of Servier Canada Inc.  This offer may not be published in any other location or refund publication without the written permission of Servier Canada Inc. Duplicate requests will not be honoured or acknowledged.  Any misrepresentation or fraudulent information, including the use of fictitious or multiple names or addresses, disqualifies this claim and may result in submitter being subject to civil or criminal liability.  We reserve the right to request additional information regarding this rebate, and the right to confirm identification.  Information collected on this form will be used to administer the on-line redemption.
