Provide Original Sales Receipt:  Circle the Waterpik Product Purchased, Purchase Date, and Purchase Price. 
Cut out UPC (purchase seal) from Box.

Items must be received by January 31, 2025.  Limit four rebates per household.  Cannot be used in combination with other offers.  Offer valid from 1/1/2023 - 12/31/2024.
